전반적으로 기립성저혈압을 앓는 사람들이 이 같은 증상이 없는 사람 보다 심부전이 발병할 위험이 1.54내 높았으며 고혈압이 미치는 영향을 배제한 후에는 1.34배 높은 것으로 나타났다

대한대장항문학회(회장 김영진)는 `대장앎의 달`인 9월을 맞아 서울대병원, 서울아산병원, 삼성서울병원, 서울성모병원, 세브란스병원 등 5개 대학병원에서 2005~2009년 대장내시경과 위내시경 검사를 받은 51만 9866명을 조사한 결과, 몸에 이상을 느끼고 한 후에야 외래를 방문해 대장내시경으로 대장암 진단을 받은 환자들(10만 895명)중 3~4기 후기진행암으로 진단받는 비율이 무려 51.6%에 달했다. 1기 조기암은 19.9%에 그쳤다. 이에 반해 위암은 3~4기 후기진행암이 28%, 1기 조기암이 61.3%로 나타나 대장암과 대조를 보였다.

건강검진센터 방문 환자들의 경우 대장암이 위암보다 진단율이 2배(대장암 0.37%, 위암 0.19%) 가량 높았다.

그러나 병기 추적이 가능한 환자(33만 206명)를 대상으로 후기진행암(3~4기)의 비중을 보면 대장암이 위암에 비해 2.7배(대장암 20.9%, 위암 7.7%)나 높게 나타났다.

이는 대장암이 위암에 비해 정기검진으로 발견할 수 있는 확률이 높지만, 검진이 늦어질 경우에는 위암보다 진행이 더 빠른 것으로 볼 수 있는 대목이다.

전체 환자를 병기별로 보면 대장암은 1기(39.8%)>0기(24.9%)>2기(14.4%)>3기(12.9%)>4기(8.0%) 등의 순을 보였으며, 위암은 1기(85.0%)>3기(4.3%)>2기(4.0%)>0기굛4기(3.4%) 등으로 위암의 조기암 조기암 진단비율이 대장암보다 크게 높았다.

대장암이나 위암 모두 건강검진을 통해 암을 진단받은 환자들(대장암 20.9%, 위암 7.7%)에 비해 3~4기 진행암 진단율이 2~3배나 높았지만, 1기 조기 진단율만 놓고보면 대장암의 악화속도가 더 빠르다는 것을 한눈에 알 수 있다.

대장암은 암세포가 점막층에 국한된 0기에 발견될 경우 간단한 대장내시경 수술만으로도 완치율이 거의 100%에 달하며, 1기에도 완치율이 90% 이상이다. 하지만 완치율은 후기 진행암으로 갈수록 낮아져 4기의 경우 통상 5% 이하로 보고 있다.

상황이 이런데도 건강검진 수진자 중 대장내시경을 받은 사람(11만228명)이 위내시경 검사자(40만 9638명)의 4분의 1 수준에 그치고 있다.

학회 김남규 이사장(연세의대)은 "조기 검진으로 대장암을 예방하려면 적어도 50세부터 대장내시경을 받아야 한다"면서 "50세 이상 성인은 5년마다 대장내시경을 받도록 하고, 대장암 가족력이 있거나 용종, 염증성 장질환, 유전성 암 등으로 진단받은 경우에는 이보다 훨씬 젊은 나이부터 검사를 받는 게 좋다"권고했다.

보건복지부는 2005년 9월 1일부터 암환자의 고액 진료비 부담을 완화하기 위해 '암환자 산정특례제도'를 시행하고 있다. 즉, 암 확진을 받아 등록한 환자가 5년간 암으로 외래·입원진료 및 합병증 치료를 받은 경우 건강보험이 적용되는 진료비의 5%만 환자가 부담하고 나머지는 국민건강보험공단에서 부담하는 것이다. 문제는 산정특례제도가 암환자에게 5년간만 한시적으로만 적용되기 때문에 2010년 8월 31일을 기점으로 최초 등록한 24만여 명의 암환자들이 그 대상으로부터 제외된다는 것. 그렇게 되면 암환자는 건강보험 진료비를 입원의 경우 20%, 외래의 경우 30~60%(의원 30%, 병원 40%, 종합병원 50%, 상급종합병원 60%)을 부담해야 한다. 이에 따라 지난 8월 10일 보건복지부는 '등록기간 종료에 따른 암환자 산정특례 운용방안'을 발표했다. 그 내용은 2010년 8월 31일자로 5년을 경과한 암환자들의 산정특례 혜택은 자동 종료되며, 잔존암이나 전이암이 있을 경우에만 재등록을 통해 혜택을 받을 수 있도록 한다는 것이다. 본인부담 5%에서 60%...암환자들 '막막' ▲ 등록 종료에 따른 암환자 산정특례 등록 안내 ⓒ 안기종  암 암의 경우 5년이 아니라 10년이 지나서도 얼마든지 전이나 재발이 있을 수 있고 전이나 재발된 암의 치료에 대해 최선의 치료 결과를 얻기 위해서는 그 여부를 최대한 빨리 발견하는 것 이외에 별다른 방법이 없다.  5년 이후 암 전이나 재발 여부에 대한 정기적인 추적검사는 환자가 원해서 하는 것이 아니라 의학적 소견을 바탕으로 한 의료진의 권고에 의한 것이다. 그럼에도 5년 경과 암환자의 산정특례를 중단하는 것은 암 투병으로 경제적인 여유가 없는 수십만명의 암환자와 그 가족들을 외면하는 것과 다름 없다. 2005년 7월 신우암 판정을 받고 좌측 신장과 방광 일부를 제거하고 총 20회의 항암치료를 받은 김지회(41)씨도 8월 31일자로 산정특례 대상자에서 탈락된다. 대학병원을 다니는 김지회씨는 앞으로 건강보험 진료비의 60%를 외래진료비로 내야 한다. 기존 5%에 비하면 12배나 더 많은 진료비를 지불해야 하는 것이다. 김지회씨는 신우암 재발 여부 확인을 위해 매년 1회 양전자 단층촬영(PET)을 하고 있다. 기존에는 산정특례로 인해 양전자 단층촬영(PET) 검사비로 약 5만원만 지불하면 되었는데 앞으로는 약 60만원을 내야 한다. 김지회씨는 양전자 단층촬영(PET)를 위해 55만원을 추가 부담해야 한다. 김지회씨의 수입으로는 4인 가족 생활비과 자녀 교육비로도 빠듯한데 55만원의 추가비용은 가계에 큰 부담이 된다. 이번 보건복지부의 발표 중에서 가장 문제기 되는 것은 암 등록 후 5년까지는 암과 관련한 합병증 치료시 산정특례를 인정하지만 5년 이후의 암 합병증 치료는 산정특례를 종료시킨 것이다. 암 치료의 대부분은 항암치료, 방사선치료, 장기이식 후에 발생하는 각종 합병증을 치료하는 것이다. 산정특례를 적용함에 있어 합병증의 경우 5년 전과 5년 후를 구별할 합리적 이유가 없다. 5년 경과할 때까지 합병증이 낫지 않아 5년 이후에도 계속적으로 치료를 받아야 하는 환자라면 경제적 부담은 더욱 가중될 것이 분명하고, 이러한 환자에게 오히려 산정특례의 유지가 더더욱 필요하다. 2005년 8월에 백혈병으로 골수이식을 받은 최민석(가명, 38)씨는 이식 합병증인 만성 숙주반응으로 5년이 지난 지금까지 치료를 받고 있다. 그도 8월 31일자로 산정특례가 종료된다. 그는 현재 매일 면역억제제(2알)와 골다공증약(1알)을 복용하고 있다. 그리고 2개월마다 외래진료에서 진찰과 혈액검사를 받고 있다. 또 매년 각종 합병증 관련 검사와 백혈병 재발 확인을 위한 유전자검사를 받고 있다. 최민석씨는 작년 한 해 동안 비급여를 제외하고 건강보험 적용되는 진료비로 약 30만원을 지불했다. 그러나 역시 대학병원을 다니는 그도 9월 1일부터는 건강보험 진료비의 60%를 부담해야 하기 때문에 년간 30만원의 12배인 360만원을 진료비로 내야 한다. 여기에 비급여 진료비까지 합하면 수백만원에 이를 것이다. 암 조기발견에 발 벗고 나선 정부가 왜... ▲ 암환자 진료비영수증 암환자는 고액의 치료비 부담으로 5년 경과 후에도 경제적 고통이 계속됨 ⓒ 안기종  진료비 암환자에 대한 산정특례을 축소하는 보건복지부의 이번 조치는 암환자의 진료비 경감을 위한 혜택을 점진적으로 증가시키고 국민건강보험 보장성을 확대해야할 국가 의무를 다하지 못한 조치이다. 이는 복지 혜택 확대를 통한 양극화 해소에 대한 국민들의 염원을 무시하는 처사이며, 암 치료로 극빈층으로 전락하고 있는 수많은 환자와 가족들의 건강권을 더 이상 보장하지 않겠다는 것과 다르지 않다. 암 조기발견을 위한 무료검진을 확대해 치료성적 향상에 애쓰고 있는 현실에서, 정부가 나서 막상 전이나 재발의 위험이 일반인에 비해 월등히 높은 5년 경과된 암환자에 대한 혜택을 줄이는 것은 누구도 이해하기 힘든 황당한 처사이다. 정부는 5년 이상 투병하는 암환자의 합병증 치료와 의료적 판단에 의한 추적검사에도 산정특례를 계속적으로 유지해야 한다. 더 나아가 정부는 건강보험 보장성을 획기적으로 확대해 암환자의 진료비를 대폭 경감하는 적극적인 조치를 취해야 할 것이다. 덧붙이는 글 | 안기종 기자는 한국백혈병환우회의 대표로 일하고 있습니다.
